Dr Eric Bergstrom is great. I have a kid who wears strong prescription glasses, so we go often. They've been a fantastic source of care and training (for contacts).

New Patients

During your first eye exam, Dr. Bergstrom, Lo, or Urbaniak will give your eyes a thorough examination and then answer any questions you may have.

Arrival Instructions: To save time at check-in, please submit your paperwork online prior to your appointment. If you are unable to submit online paperwork, please arrive 10- 15 minutes early for your appointment.

Please arrive with all current eyewear, including boxes of contact lenses. Contact lens wearers, please arrive wearing glasses unless you've received trial lenses and are scheduled for a contact lens follow-up.

WHAT SHOULD I BRING TO MY EXAM?

Please bring your most current glasses or current contact lens boxes, plus your insurance card.

HOW LONG IS A TYPICAL EXAM?

Your exam will typically take 30-45 minutes, depending on the complexity of your particular situation and any questions you may have.

WILL MY PUPILS BE DILATED?

Drs. Bergstrom, Lo, and Urbaniak prefer to dilate new patients, but it can be delayed or postponed depending on your schedule.

I CURRENTLY WEAR OR AM INTERESTED IN WEARING CONTACT LENSES. WHAT SHOULD I EXPECT?

To be prescribed contact lenses, the doctor will perform an additional evaluation. This involves calculating and choosing the proper lens type and prescription specific to your contact lenses, as well as additional evaluation of your eye health to determine whether you are a suitable candidate for beginning/continued contact lens use. The doctor must also verify that you are able to insert and remove contact lenses on your own before prescribing contact lenses. If you're a first time wearer, or have difficulty inserting and removing your lenses, a training will also be required.

SHOULD I COME IN WEARING MY CONTACT LENSES?

The doctors prefer for new patients to arrive wearing glasses. It will make the examination process more efficient.

CAN I BRING MY CHILD/CHILDREN?

Yes, as long as you don’t think their presence would be a distraction for you.

IS PARKING AVAILABLE?

Yes, free parking is available in front of the building during office hours. Free street parking is also available near the clinic.

DO I NEED AN APPOINTMENT TO WORK WITH AN OPTICIAN?

No, our opticians work on a walk-in basis, and there is always someone on staff to assist you.
